Prednisolone Side Effects Explained: Mood Changes, Irritability, Restlessness, Sleep Problems and Behavioural Changes — and Why Family May Notice First 💊 “Raj, the prednisolone is working.” Then his wife quietly said: “But he isn’t himself anymore.” That sentence matters because prednisolone doesn’t only cause physical side effects. It can also affect sleep, mood, thinking and behaviour. UK prescribing information for systemic prednisolone describes reported effects including: • difficulty sleeping • restlessness • irritability • anxiety • feeling unusually high or euphoric • low or rapidly changing mood • behavioural changes • confusion • and, less commonly, more serious psychiatric symptoms. And these changes can sometimes appear within days or weeks of starting systemic corticosteroid treatment. Higher doses may increase the risk, although the dose alone cannot reliably predict who will experience a reaction or how severe it will be. That is why someone close to you can sometimes notice the change before you do. Perhaps you think: “I’ve just got more energy.” While your partner notices: “You’re hardly sleeping.” “You’re unusually restless.” “You’re snapping at everybody.” “You don’t seem like yourself.” That does not automatically mean prednisolone needs to be stopped. Two things can be true: ✅ Prednisolone can be controlling inflammation and helping the condition it was prescribed for. AND ✅ It can be contributing to changes in sleep, mood or behaviour. The NHS lists insomnia, restlessness and mood changes among recognised prednisolone side effects. So if something changes after starting prednisolone, increasing the dose, or during treatment, tell the person reviewing your medicine. A useful question is: “Could my prednisolone be affecting my sleep, mood or behaviour?” And involve somebody who knows you well if possible. Their observations can be useful because sometimes the person experiencing the change does not recognise it immediately themselves. ⚠️ DO NOT suddenly stop or change prednisolone yourself. Stopping can be dangerous in some circumstances, particularly after longer treatment or higher doses, because the body may have reduced its own steroid production. Treatment often needs to be reduced gradually when stopping is appropriate. And there is an important safety distinction. Mild insomnia or irritability deserves review. But worrying or severe psychological changes require prompt medical advice. The MHRA advises patients and carers to seek urgent medical advice if worrying psychological symptoms develop, particularly symptoms such as suicidal thoughts.
