One of the biggest misconceptions about women’s sexual health is that orgasm is “all in your head.” The reality is much more nuanced. A recent study on the journal: Obstetric Gynecology, in August 2026: “Female Orgasmic Disorder: Current Understanding and Clinical Management”, found that orgasmic difficulty affects up to 1 in 4 women, and it’s rarely one single cause. Hormones, medications like SSRIs, nerve function, pelvic floor muscles, clitoral anatomy, medical conditions, relationship dynamics, and mental health can all play a role. The clitoris is the primary driver of orgasm for most women, and yet so much of the research, and the conversation around it, has lagged behind. That’s why a thoughtful evaluation matters: a real sexual history, a physical exam that actually looks at the anatomy involved, and sometimes bloodwork to rule out hormonal or thyroid contributors.
